What registrations and other things do I need in order to due a thru hike?

You need to buy a permit for the Smoky mountain NP, no more then 30 days before you expect to get there. There is a voluntary registration at the Appalachian Trail Conservatory web site should you wish to add to the data base. That will give you an idea of how many other people you'll likely to see the day you head out.

you only need to pay for a permit for the smokys. no registration required to walk the trail

The Appalachian Trail Conservancy has asked that thru hikers register voluntarily, however you are not required to do so. http://www.appalachiantrail.org/home/explore-the-trail/thru-hiking/voluntary-thru-hiker-registration
Beyond that there are three places: Great Smokey Mountain National Park, Shenandoah National Park, and Baxter State park, that require hikers to register (or at least sign in).
