Chafing

I don't think this is going to be a very pretty thread, but it's a topic that seems to be mentioned a lot.

What is chafing, what causes it, and how can you avoid it?

I've never done a distance hike, but during hikes (usually 2-4 days long), I have never had any problems that point to chafing.
