Does the Pacific Northwest Trail connect the Pacific Crest Trail and the Continental Divide Trail? and has anyone attempted to hike all 3 in a single attempt?

Yes. Look at Andrew Skurka's great western loop of a decade or so ago.

Legend is currently hiking the Western Loop. He has finished those three trails and just started on the Grand Enchantment trail.

The GWL as Andrew accomplished it doesn't complete all of the CDT, PCT, PNWT,...between established termini if that is desired.
